Rules For Anniversary Date Changes
BCBSIL Off Cycle
- Paperwork due 30 days PRIOR to requested anniversary date
- Paperwork needed:
- Signed letter from the group on company letterhead with request
- BPA and BPS
- EE Apps (Required for anyone changing plans or coming onto one of the plans)
- All rates will change – new rates will be based on new Anniversary Date & census at time the UW processes the paperwork. (Rates are not determined by anticipated enrollment)
- There are NO restrictions on what time of year an anniversary date change can be done.
- Triggers open enrollment period for all employees who have met the new hire waiting period

- New rates, new plans, new billing method, reset renewal date, and more.
